RECOGNITION OF PATHOLOGY DEPARTMENT WITH PMDC & CPSP

Pathology Department, Sheikh Zayed Medical College, Rahim Yar Khan has been recognized with the Pakistan Medical & Dental Council w.e.f. 12-07-2006. Most importantly, Postgraduate teaching programme has been started since 2010. Haematology and Chemical Pathology sections of this department have been accredited for FCPS-II training programme.

Reception Pathology Laboratories:

Reception area for registering patients, collection of samples and delivery of reports is just next to the Pathology Laboratory in the separate building. This part has maximum space for the patients sampling. There are three different areas separately for the sampling, registration and delivering of reports. Fully computerized laboratory information system is in process.

iii) Practical/Demonstration Laboratories for Students:

Pathology Department started providing its teaching services in new constructed boiling of Sheikh Zayed Medical College, Rahim Yar Khan with effect from November 2017. In this new building, Pathology Department is consisting of following four spacious Practical/ Demonstration Laboratories for students with adequate instructional aids/equipments and reagents. These laboratories are well established and properly equipped.

iv) Seminar / Tutorial Room

Seminar room of this department is also established for Case Discussions, Presentations, Journal Club Meetings and to meet the day to day advances in Medical Sciences with the facility of LED and Multimedia

Pathology Museum

Pathology Museum is a very essential part of the Pathology Department. This is situated at a prime location that is in easily accessible for both teachers and students. At the time of beginning of the college, there were only 50 specimens in this museum but our continuous efforts the specimen numbers have increased upto 170. These specimens are adequately preserved, labeled in glass jars. This diverse collection is lying in wooden almirahs. All almirahs are filled with properly labeled pathological specimens. The system vise labeling of the almirahs have been done for easy approach to a specific specimen. 15-20 medical students can be accommodated comfortably around a set of two tables placed in the center of museum for discussion on the specimen.

A departmental library with adequate number of essential books and periodicals/journals are kept in it for the help and benefit of teaching staff. This helps the teachers to abreast them with the rapid intervention on the subject and attaining a grip and confidence in their respective field.

ACHIEVEMENT OF THE DEPARTMENT

Two MBBS classes “3rd year MBBS & 4th year MBBS”, four classes of B.Sc. Medical Laboratory Technology (MLT), in addition to classes of Doctorate of Physiotherapy (DPT), B.Sc. Operation Theatre Technology (OTT) & B.Sc. Medical Imaging Technology (MIT) are under the teaching of the Pathology Department.

(A) MICROBIOLOGY SECTION

Microbiology is a broad discipline which has both basic and applied aspects. There is a continual process of technological advances in the field of microbiology. The clinical microbiology laboratory at Sheikh Zayed Hospital Rahim Yar Khan was established in 1997 under the supervision of Prof. Dr. Abdul Hakeem. It is one of the oldest section in the Pathology Department. Now Microbiology Section has a highly qualified faculty for teaching and training of students of MBBS and Allied Health Sciences. The faculty comprises Associate Prof. Dr. Muhammad Shahbaz Hussain, Head of Pathology Department, Dr. Umar Khalid Cheema Assistant Professor, Senior Demonstrator, Medical Officers and Medical Technologist.

Teaching:

“Education is improving the lives of others for leaving your community and world better than you found it”. Undergraduates of the MBBS programme, undergraduates of Allied Health Sciences and postgraduate students are taught basic microbiology, immunology, common infectious disease and hospital infection control. In addition, the students experience hand on integrated practical training also. As a results, teaching methodology, examination question reflect this aspect. Hands on Training Workshop on Biosafety and Quality Control have been conducted with American Society of Microbiology (ASM).

Research:

The section of Microbiology has a dynamic profile. Under high profile academic leadership the section is involved in many scientific projects related to bacteriology, impact of herbal natural plant, on resistant bacterial and increasing burden of antimicrobial resistance.

Services:

The section of Microbiology has a state of art diagnostic bacteriology, mycology, parasitology, serology and urine analysis subsection. A well automated equipped microbiology laboratory is functional round the clock. Trained laboratory personnel perform the following tests:

  • Routine Microbiology & Serology Tests
  • ELISA (HBV, HCV, HIV & Dengue)
  • Serology of VHF
  • PCR and Genotyping (HBV, HCV)
  • Gene-Xpert for diagnosis of MDR cases of Tuberculosis
  • Culture and Sensitivity of all clinical samples

Recently more facilities in culture and sensitivity laboratory comprising of most advanced automated Microbial identification and anti-Microbial susceptible Testing System with Bectec-Alert 3D and Vitek-2-Compact has been installed that provide highly accurate and reliable results.

  • Biological Safety Laboratory (BSL) Level-II & Level-III has started performing tests for Covid-19 successfully.
  • In near future, MG IT, Anaerobic Culture and full range of Fungal Cultures will be started in the section.

The Section of microbiology also provides consultancy for diagnosis & treatment of infectious diseases for inpatients and outpatients to bring management in line with laboratory finding. The section also provides consultant opinion to critical case areas (ICU, NICU, HDU in wads CCU) for infection control problems to monitor the patient’s investigations and management of outbreak.

MOLECULAR BIOLOGY

i) PCR

The Polymerase Chain Readction (PCR) is considered as one of the most revolutionary method in the field of molecular biology. It is the only technique that can clearly asses molecular therapeutic milestones. It allows detection of low copy targets with greater precision and confidence. We use PCR amplification for the diagnosis and prognosis of infectious diseases. It employs detection of viral RNA for Hepatitis C virus and detection of viral DNA for Hepatitis B virus. PCR Lab was established at Sheikh Zayed Hospital, Rahim Yar Khan in 2009 under the supervision of Prof. Dr. Muhammad Abdul Rehman. Now PCR section is continuously excelling its services under the kind supervision of Prof. Dr. Muhammad Shahbaz Hussain, Assistant Prof. Dr. Nabila Rauf, Senior Medical Technologist Mr. Muhammad Ali Malik, Biochemist Mrs. Nadia Usman and Senior Lab. Technicians.

PCR Lab. is equipped with state of art Real time PCR instruments, Rotor Gene Q Real time PCR system by Qiagen Germany and Sa-Cycler 96 Real time PCR System by sacace biotechnology, Italy. Real time PCR system is the latest and confirmatory method for diagnosis of Infectious diseases. It also serves as a solid foundation for quantitative measurements in a true sense. PCR section is also equipped with latest and up-to date automated extractor Genti- Tm 32 by Korea.

The section is also providing the services of Medical Fitness for the confirmation of their results. The section is providing the services of bench work for the students of BS Allied Health Sciences and internees and also arrange short training courses for the junior staff to promote requisite expertise in the concepts, approaches and basics of PCR technique.

Future Plans:-

To upgrade the department by providing state of the art lab. Facilities for advance research and practical training.

ii) BSL-III LAB.

Biosafety Level-III Laboratories are mostly used to study infectious agents or toxins that may be transmitted through the air and cause potentially lethal infections.

Biosafety Level-III Laboratories describe the application of specific practices, safety equipments and specially design laboratories to create a safe environment both within and outside the laboratory, for work conducted through infectious agents and toxins.

To fight against the pandemic of COVID-19, BSL-II & III Lab has been established at Sheikh Zayed Medical College/Hospital, Rahim Yar Khan and by the grace of Almighty Allah, the Biosafety Level-III (BSL-3) Lab. has started to provide its services from 01-02-2021 for COVID PCR Testing of District Rahim Yar Khan. This Lab is under supervision of Head of Pathology Department and run by qualified and professional Medical Technologists and Biochemists. More than one lac tests of COVID-19 have been performed in this lab up till now.

(B) HEMATOLOGY SECTION AND BLOOD TRANSFUSION SERVICES

In this developing era of medical science conventional and advanced haematology is contributing a great impact with respect to the diagnosis and management of malignant and non-malignant haematological disorders. In this context the application of immunological, cytogenetics and molecular diagnostic methods has brought modalities in the treatment protocol. Besides the conventional diagnostic instruments, automated haematology analyzer, coagulometer, and Hb Electropherosis moved this field into advanced scientific diagnostic applications. Bone marrow aspiration/trephine biopsy, Hb electrophoresis and coagulation profile other than routine Haematological diagnostic tests are outcome of dedicated efforts of Associate Professor & Incharge Haematology    Dr. Muhammad Bilal Ghafoor. The institute is distinguished from all other medical colleges in having an Academic Blood Bank (established 2010), who revolutionized the trends of Transfusion medicine in this area and introduced the bright concepts of component therapy in transfusion, is running under the supervision of supervisor blood bank providing facilities of blood component preparation and their therapeutic applications with respect to different haematological conditions. Mrs. Shamraiza Zulfiqar has been a cornerstone of this facility who is associated with the very beginning of it. The section is fulfilling all the requirements for Postgraduate Training to produce skilled and competent Hematologists. In this context, the Department of Haematology has been accorded full accreditation by CPSP for FCPS Part-II Training in Haematology in 2011. Regular case presentation, group discussion, bench work and interactive physician forum meetings are held under the guidance and supervision of Haematologist to enhance the subjective and objective cognitive and psychomotor skills amongst the Post graduate Trainees. Recently a fully equipped Thalassemia Centre has been established adjacent to Blood Bank by day and night devoted efforts of Associate Professor Dr. Muhammad Bilal Ghafoor and Ex. Professor   Dr. Atifa Shuaib. This has overcome miseries of poor and needy patients of Thalassemia coming from local and far flung areas of Sindh & Baluchistan .Dr. Muhammad Bilal Ghafoor Associate Professor Hematology is the focal person for “Centre for Thalassemia Care”. He manages the center, examines and treats the thalassemia children. The Hematology division of Pathology Department is arranging regular workshops and awareness programmes for prevention & awareness of thalassemia and common hematological illnesses.

The Hematology Section has been upgraded under supervision of incharge Hematology Dr. Muhammad Bilal Ghafoor by addition of fully Automatic Coagulation Analyzers, Five Part Hematology Analyzer, 7 Part Hematology Analyzer & Fully Automatic HB Electrophoresis Machine to provide best services to mankind. Dr. Sana Khan (FCPS Haematology / Medical Officer), Dr. Samrah Abbasi (Medical Officer), Dr. Farah Yasmee (Medical Officer), Dr. Aqsa Khalid (Demonstrator), Mrs. Shamraiza Zulfiqar (Medical Technologist) and Mrs. Faiza Sarwar (Medical Technologist) are working efficiently to provide best hematology lab. services. The blood bank has also been upgraded by installation of Automatic Cross Match, Automatic Blood Grouping, Three Cell Panel and 11 Cell Panels, a step towards modern blood banking. Platelet aphresis and Plasma aphresis are planned in near future.

The departments is involved in research work & paper publication. The department is moving forward to establish institute of Haematology so that the trained Hematologist can be produced to meet the need of the region.

(C) CHEMICAL PATHOLOGY SECTION

Chemical Pathology is an ever-expanding and dynamic field of biological science. The department of Chemical Pathology is working under the supervision of Dr. S. Sabahat Haider, Associate Professor (FCPS CHEM-PATH). The department has been recognized for FCPS-II and MCPS training by CPSP. There are currently 03 PGRs of FCPS-II Chemical Pathology, Dr. Rukhsana Tumrani & Dr. Anber Rahim & Dr. Tayyeba Younis and 02 PGRs of MCPS Clinical Pathology, Dr. Maria Masroor & Dr. Farah Yasmeen. The addition of Dr. Mehvish Sana, FCPS Chemical Pathology / Medical Officer in Chemical Pathology Section increases the strength of diagnostic services. The dept. has one Medical Technologist Mrs. Rabiya Mehmood. Chemical Pathology Section has taken up the challenge to make the upmost utilization of clinical chemistry accessible and understandable through organizing comprehensive training programs for undergraduate and postgraduate trainees and equipping the laboratory with the latest automated analyzers and electronic instruments.

Chemical Pathology is a discipline of Pathology which has maximum workload of diagnostic laboratory tests pertaining to the diagnosis of the diseases. The workup in the section is helpful not only to the clinicians but also essential to understand the pathophysiology of various diseases for the undergraduate & postgraduate medical students.

The diagnostic work is being done on Fully Automated Analyzers in Chemistry Lab i.e., Fully Automated Chemistry Analyzer, photometric / turbidimetric, (AU 680) with throughput of 1200 tests/hour which performs routine chemistry tests, Fully Automated Chemistry Analyzer (BA 400) on which emergency chemistry tests are performed with through put of 300-500 tests/hour. There is also one Chemiluminescence based “CLIA’ (Acess-2) Hormone Analyzer, Electrochemiluminescence based “ECLIA” state of the art (e 411), hormone analyzers to carry out estimation of hormones and tumor markers, Standalone dedicated Direct ISE based electrolyte analyzers for serum/body fluids electrolyte analysis, Standalone dedicated Arterial blood gas analyzers are also available in the section but currently placed in the cardiac surgery ICU and Emergency for the facilitation of physician and patients, Three RO based water purification systems for the continuous availability of Reagent Grade Water for all the automation tests especially Na, K, Cl.

All these facilities play a vital role in both the academic and research fields. With the availability of such advanced equipments it has become possible to diagnose/confirm most of the diseases in shortest possible time. The clinicians and researchers can make best use of such facilities.

  • Pathology Department successfully conducted the ChemCon2020 Conference “11th ChemCon 2020 Conference of Pakistan Society of Chemical Pathologists     (09-10 April 2020)” and Workshop “Point of Care Testing in Primary Critical Care (09th April 2020)”.

Allied Health Sciences

This department also plays vital part in carrying out the various research projects of not only the postgraduate trainee/consultant/specialist but also hosts the “Allied health sciences” students and currently Medical Laboratory Technology (MLT) students are also carrying out their research under supervision of Associate Professor Dr. S. Sabahat Haider.

Certainly this serves an extensive understanding of our present and future scientific, technological, and organizational development. The practical work carried out in the laboratory and the ongoing teaching programs in this discipline are not only for the education of those getting training in clinical laboratory as professionals but also for the continuing education of those who wish to acquire specific expertise and recognition.

(D) HISTOPATHOLOGY & CYTOPATHOLOGY SECTION

It was founded in 22-07-2005 and presently running under the supervision of Dr. Muhammad Shahbaz Hussain, Associate Professor and Head of Pathology Department. Other faculty members are Dr. Sadia Khalid, Demonstrator and Dr. Ayesha Zahoor, Medical Officer. Section has a well equipped laboratory and a diverse Pathology Museum. Our Pathology Museum has variety of gross specimens. All these facilities are enormously helpful for training of under graduate and postgraduate students. Histopathology Lab. is equipped with modern apparatus which includes Automatic tissue processor, Tissue Stainer, Microtome, Knife Sharpener etc. Here utmost care is taken from the first step of sampling and preservation to the final stage of reporting.

Biopsy specimens are being reported in this section in addition cytological examination of fluid & FNAC (Fine Needle Aspiration) is being performed here. We have specimens and slides for demonstration to students of 3rd Year and 4th Year MBBS for practical purposes. Students of 2nd Year B.Sc (Hons) Medical Laboratory Technology (MLT) & B.Sc DPT 3rd year have been trained in Histopathology Section. The Postgraduate Trainees of Dermatology and Paediatric Surgery have completed their rotational training in this department.
